Job summary

Excellence Care Ltd is hiring a dedicated full-time Support Worker to provide person-centred care on-site in the UK. You will support adults with mental health needs, learning disabilities and complex needs, following individual care plans and promoting independence and wellbeing.

Role involves assisting with daily living, medication, community access and social activities, working with team leaders and care managers.

Qualifications

• Previous Support Worker experience preferred but not essential.
• Must be caring, dependable, patient and committed to making a positive difference.
• Full UK driving licence desirable but not essential.

• Support individuals with personal care, medication and daily living.
• Encourage independence, choice and decision-making.
• Support individuals to access the community and develop social relationships.
• Promote healthy lifestyles and wellbeing.
• Work closely with Team Leaders, Care Managers and other professionals.
• Contribute to person-centred plans, reviews and support goals.
• Act as a Key Worker where required.

What We Are Looking For
