Job description

The Support Worker is an experienced professional dedicated to thesupport services for individuals in need. Serving as a beacon of inspiration and motivation, they support residents through the intricacies of providing compassionate and effective care. Their responsibilities extend to assessing individual needs, formulating comprehensive care plans, and offering mentorship to our residents.

Requirements:

To be able to undertake sleep in duties as required

To be able to work shift patterns including evenings, weekends, bank holidays as required

A can-do attitude that supports the company’s direction and growth

Key Responsibilities:

Nurturing:

Inspire and motivate residents to help them achieve their long-term life goals.

Care Planning:

Develop and implement personalised care plans for individuals under support.

Assessment: Conduct thorough assessments of individuals' needs and adapt care plans accordingly.

Guidance:

Provide guidance and support to residents, ensuring adherence to house rules.

Coordination:

Collaborate with external agencies and healthcare professionals for holistic support.

Compliance:

Ensure strict adherence to relevant regulations and standards.

Training:

Participate in and facilitate training and development programs for continuous improvement.

Main Duties & Responsibilities:

To undertake a mentoring role in ensuring the care and welfare of each resident in placement

To give guidance and support to all residents

To assist the Senior Support Worker with daily residential tasks

To provide key work sessions every week (minimum) with all residents

Ensure the care/placement plan for each young person is always followed and adhered to

To ensure education is promoted, supported and that each resident’s attendance in education is maintained.

To ensure that all staff act in an appropriate manner

To ensure that the home's policies and procedures are always adhered to

To ensure all risk assessments are consistently and robustly applied

To ensure that all safeguarding concerns are reported to the Designated Safeguarding Lead/Head of Operation immediately

To read and familiarise with all the daily logs once on shift

To attend meetings and training as required

Undertake any other reasonable tasks deemed required as directed by the Senior Support Worker or Management

At least one year of experience working in a residential setting with young people with complex and challenging behaviour.

Current Driving Licence

Ability to multi-task and meet deadlines

Education, Training and Qualifications:

Any health care related NVQ Level 2 and above

Relevant health care short courses

Be prepared to complete training courses
