Main duties of the job

As part of the Core Adult Mental Health Team you will develop strong links with carers and other networks to support the service user to develop a co-produced management plan that allows them to manage their mental health and build on existing skills and community links to further support their future management and wellbeing.

The support worker will be supervised by Senior Mental Health Practitioners and will be closely supported as necessary in the management of risks and and any potential decline in a person's mental state by the Core Team.

The support worker will have clear pathways of escalation and will work within the bounds of existing DPT policies and procedures as required.

The Post Holder will be able to demonstrate excellent communication skills with both services users, carers and organisations. You will be highly motivated and display a good ability to work autonomously where possible.

The support workers will work with service users with the full range of mental health diagnoses who find themselves under the care of the Core Adult Mental Health Team who will provide clinical direction and support.

The support worker will be proactive in seeking out the positive support networks available in the community for each individual which may help them in their recovery.
