Changing Lives in Doncaster is seeking a Support Worker for Open House Plus, a 26-room supported accommodation for people experiencing homelessness. You will join a committed team delivering information, advice and practical support to help residents live independently and access essential services.
You will receive training on all aspects of the role and participate in case management reviews with the whole team.

Open House Plus, located in Doncaster, is a 26 room supported accommodation for people experiencing homelessness. Our project provides supported accommodation for individuals with a number of complex needs. As a Support Worker at Open House Plus, you will have the opportunity to join our passionate team who are dedicated to empowering those who we support. We are committed to delivering high quality support, and we welcome your expertise and dedication in order to help us to continue making a positive impact in the lives of those we support. You will be given training on all aspects of the role as well as taking part in case management reviews with the whole team.Our project operates on a shift schedule; therefore, flexibility is essential as shift times may vary to ensure uninterrupted support.
The Support Worker plays a crucial role within Changing Lives, providing information, advice and guidance, which is undertaken via referral and direct individual support. The Support Workers aim is to empower the people we support to live independently and work towards overcoming the interconnected problems that lead them into accessing our services.
Your key activities will include:
Working both individually, and as part a team, to build productive relationships with individuals accessing the Changing Lives service.
