Location: Community Work, based at Queen Margaret Hospital, Fife. Travel around the Dunfermline area.
Hours: To be specified per week.

Our new discharge to assess service in Dunfermline enables people, following a stay in hospital, to be supported and assessed in their own home to determine the type and frequency of any care and support they may need to live as independently as possible at home. Our discharge to assess service conducts these assessments in the home. Our goal is to ensure people receive the right care, in the right place, at the right time. We support and facilitate access to resources, empowering people to regain independence and confidence while maintaining dignity.

This is a Regulated Service and you will play a vital role in providing practical and emotional assistance to individuals in their own homes, assessing their needs, and conducting assessments to determine ongoing care needs.

Wondering about a day in the life of a Support Worker?

1. Conduct person-centered assessments in the individual’s home following a hospital stay. These assessments ensure that people receive the right care, in the right place, at the right time.
2. Work closely with individuals, their families, and other professionals to create tailored care plans.
3. Assist and enable individuals with activities of daily living, including bathing, dressing, meal preparation, medication management, personal care, and mobility support.
4. Collaborate with healthcare professionals, social workers, and team members for a coordinated approach to care.
5. Monitor and evaluate individuals' progress, adjusting care plans as needed to promote recovery and well-being.
6. Maintain accurate documentation of interactions, care plans, and progress.

To be a successful Support Worker, you'll need:

• Previous experience in healthcare or social care is preferred but not essential.
• Compassionate and empathetic nature, with a strong desire to positively impact lives.
• Excellent communication and people skills, with effective teamwork abilities.
• Knowledge of person-centered care principles and a commitment to dignity and autonomy.
• Ability to work independently, demonstrating initiative and reliability.
• Flexibility to work various shifts, including evenings, weekends, and bank holidays.
• A full driving license and access to a vehicle is preferred but not mandatory.
• Registration with Scottish Social Services Council is required within months of starting.
