About The Role

Empower a caseload of service users to manage their health and wellbeing. As part of a team, you will support a staffing rota offering support to service users between the hours of 8am-10pm Mon-Sun each week. In collaboration with your team, you will actively monitor wellbeing, mental health, housing and living environment to ensure service users' journey into independence. Moreover, you will take a solution-focused and co-produced approach to supporting service users with their move-on into independent living.

Responsibilities include:
  • Empowering service users to engage effectively with community professionals such as mental health teams, social workers, housing providers, and other health, employment, and training professionals.
  • Assisting in the assessment of support needs and advocating for service users' appropriate support levels.
  • Working flexibly within a team to ensure service coverage and meet contractual obligations.
  • Developing co-produced group activities, peer support networks, and community-based approaches to help service users achieve their goals.
  • Encouraging service users to develop support networks and engage in meaningful activities, including managing volunteers and befrienders.
  • Ensuring referrals, assessments, and reviews are timely and effective.
  • Monitoring health, wellbeing, safety, and security of the service and its users, following up on concerns promptly.
  • Participating in core service activities, supporting colleagues, and managing additional tasks as they arise.
  • Maintaining accurate and confidential records in a timely manner.
  • Supporting residents in maintaining tenancy and developing independent living skills.
  • Supporting property management, repairs, and health and safety initiatives.
  • Supporting residents with rent and service charges, and managing voids.
  • Clearly articulating service aims and supporting co-production with service users.

Demonstrable core skills, alignment with our values, and a willingness to learn. Lived experience relevant to the support areas is valued and should be mentioned in your application.

  • Experience working with individuals with mental health or dual diagnosis issues, paid or voluntary.
  • Ability to learn quickly, gather, and record detailed information accurately.
  • Ability to handle challenging behaviour constructively and defuse difficult situations.
  • Solution-focused approach and collaborative problem-solving skills.
  • Optimism about change and clients' capacity to change.
  • Calm, non-judgemental, and anti-discriminatory attitude.
  • Willingness to work unsociable hours, including early, late, and weekend shifts.
  • Knowledge of safeguarding procedures and ability to address safeguarding issues appropriately.
