Job summary

Change Grow Live in St Helens is seeking a Substance Misuse Nurse (Outreach) to join a multidisciplinary team. The role involves working across outreach settings, including street homelessness projects, HMOs, and clients’ homes, with flexible starts up to 07:00 when required.

The nurse recruited into this role will work across St Helens, supporting some of our most complex service users in a range of settings, including street homelessness outreach projects, HMOs, service users’ homes, partner agency premises, and more. Due to the nature of the role working hours will need to be flexible with potential 07:00am starts when required.

Hours: Part time, 30 per week – flexible working pattern may be required to meet the needs of the Service User Group.

Contract Type:Permanent

Part-time hours at Change Grow Live are 30 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

TheSubstance Misuse Nurserole is a varied one, the successful candidate will be an integral member of a multidisciplinary team, offering specialist clinical interventions to people who use our services.

In our services no two days are the same. Every person who comes into our service is treated as an individual, so understanding the importance of a whole person approach along with being curious, a good listener, a clinically robust practitioner and having a flexible approach are essential skills.

Your role includes:

  • Undertaking physical and mental health assessments for people who access our services including alcohol assessments and Health assessments.
  • Appropriate sign posting/referral to primary care or appropriate secondary health care services for identified health issues
  • Providing education, health promotion and screening for blood borne viruses (BBV)
  • Providing Hepatitis B vaccinations when indicated
  • Contributing to CGL’s Naloxone Strategy
  • Ensuring that procedures are regularly reviewed to promote best practice and evidenced based practice in accordance with UK Guidelines on Clinical Management, NICE guidance, and CGL clinical protocols and procedures
  • Providing harm reduction advice to injecting service users, providing information and equipment and ensure all equipment is disposed of safely
  • Recording and inputting client data and information
