Job summary

Change Grow Live is recruiting a Substance Misuse Nurse for our Cheshire East service (Macclesfield/Crewe base options). This full-time position involves clinical care across delivery hubs and some community work, with a flexible base choice.

You will work in a multidisciplinary team delivering high-quality care for people affected by drugs and alcohol, with focus on person-centred recovery and trauma-informed practice. Support for professional development is provided.

Qualifications

  • NMC registered nurse with current practice.
  • Experience in substance misuse or community health is desirable.
  • Commitment to person-centred care and recovery.
  • Strong communication and teamwork abilities.

Responsibilities

  • Undertake comprehensive physical and mental health assessments.
  • Signpost to primary/secondary healthcare services as needed.
  • Provide health promotion and BBV screening.
  • Administer Hepatitis B vaccinations where indicated.
  • Contribute to Naloxone strategy and best practice.
  • Ensure care aligns with UK guidelines, NICE and CGL protocols.
  • Deliver harm reduction advice and provide educational resources.
  • Maintain accurate clinical documentation and records.

We are currently seeking aSubstance Misuse Nurseto join our welcoming and skilled Cheshire Eastteam in a full time role. The position will work across both our delivery HUBs (Macclesfield and Crewe) and include some community work, the successful candidate can choose which base.

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

Responsibilities

The Substance Misuse Nurse role is diverse, rewarding, and central to the delivery of high‑quality care within our Cheshire East service. As an integral member of a skilled multidisciplinary team, you will provide specialist clinical interventions to people accessing our services, supporting them at every stage of their recovery journey.In our service, no two days are the same. We work with people as individuals, valuing a whole‑person approach that recognises physical health, mental wellbeing, and social needs. Curiosity, strong listening skills, clinical confidence, and flexibility are essential qualities, alongside a genuine commitment to compassionate, trauma‑informed care.

Your role will include:

  • Undertaking comprehensive physical and mental health assessments
  • Appropriately signposting and referring individuals to primary or secondary healthcare services
  • Providing health promotion, education, and blood‑borne virus (BBV) screening
  • Administering Hepatitis B vaccinations where indicated
  • Contributing to Change Grow Live’s Naloxone Strategy
  • Ensuring clinical practice reflects current evidence, UK Guidelines on Clinical
  • Management, NICE guidance, and CGL clinical protocols
  • Delivering harm reduction advice to people who inject drugs, including the provision of information, equipment, and safe disposal guidance
  • Accurately recording and maintaining clinical documentation and client data
