West Suffolk NHS Foundation Trust is recruiting a Band 6 Nurse to join the Suffolk Stroke Early Supported Discharge (ESD) Team. The role covers home and community based rehabilitation during the first 6 weeks after discharge, working as part of an interdisciplinary team.

You will assess patients, support independence, travel across the county, supervise junior staff, contribute to service development and quality improvement, with opportunities for professional development and supervision.

Formación

Current NMC registration (adult) is essential.
Degree or equivalent in Nursing and post‑registration education.
Experience at band 5 with stroke/neuro care is required.
Ability to organise workload and delegate / prioritise tasks.
Good communication and ability to convey complex information.
Willingness to travel across Suffolk and contribute to weekend rota.

Responsabilidades

Provide home and community based assessment and stroke care.
Collaborate across the Suffolk Stroke Pathway for seamless transitions.
Supervise junior staff and contribute to service development and data collection.
Support rehabilitation and maximise independence for stroke survivors.
