West Suffolk NHS Foundation Trust is seeking a Band 4 Assistant Practitioner for a part-time, permanent role in the Suffolk Stroke ESD Team. The post offers opportunities to deliver specialist rehabilitation for stroke patients in the community and hospital settings.

You will work with a multidisciplinary team, supported by senior therapists, and travel across the county with a base at Allington Clinic, Ipswich. The role emphasizes development through a structured training programme.

Qualifications

• Foundation degree in healthcare or equivalent.
• NVQ level 3 with post-qualification further study.
• NVQ level 4 with work-based experience.
• GCSE Maths and English grade C/4 or above.

Responsabilités

• Deliver individualised rehabilitation care plans under supervision.
• Work with patients, families and carers within a multidisciplinary team.
• Travel across Suffolk; base at Allington Clinic, Ipswich.
• Provide support for stroke rehabilitation and documentation.
