Job summary

Northamptonshire Healthcare Foundation Trust's stroke dietetics service seeks an experienced dietitian to provide assessment and management for two wards (52 beds) within NGH. You will work with the stroke MDT to develop discharge plans and provide specialist dietary advice.

The role involves complex case work, outcome evaluation, and training for staff, carers and other professionals. NHFT promotes learning and living wage.

Qualifications

• Undertaking comprehensive assessment of patients with complex presentations and multipathologies within inpatient and outpatient settings for patients referred by the stroke team.
• Formulating individual management and treatment plans using advanced clinical reasoning and developing comprehensive discharge plans including advice on appropriate ACBS prescribable enteral feeds and supplements to be used.
• Working collaboratively with the wider multi-disciplinary health care team to ensure delivery of coordinated care and to provide specialist advice and attend case conferences, network meetings as appropriate.
• Evaluating patients' progress, measuring outcomes and contribute to and implement service improvement work to provide safe patient care and manage risk.
• Using a range of verbal and non-verbal communication tools to communicate effectively with patients to complete dietetic assessments and implement treatment plans.
• Providing advice and training to patients, carers, colleagues and other healthcare professionals to promote an understanding of the aims of dietetic treatment to ensure a consistent approach to patient care.
