Job summary

NHS Hampshire and Isle of Wight Integrated Care Board is seeking an All Age Continuing Care leader to join the senior management team. You will lead in all aspects of continuing care including children, S117, personal budgets, and commissioning, delivering a complex, integrated service with a strong collaborative ethos.

Qualifications

• Senior leadership experience in All Age Continuing Care or related health/social care.
• Ability to build trusted stakeholder relationships in a political context.
• Experience delivering complex change programmes and board approvals.

Responsibilities

• Develop strategy for NHS All Age Continuing Care across the ICB.
• Engage policy makers to inform strategy and policy development.
• Communicate the vision for All Age Continuing Care and secure buy-in.
• Identify national best practices to benefit patients.
• Drive transformation to a Place-based integrated delivery model.
• Lead and monitor delivery against national frameworks and budgets.
• Perform other duties as required by line management.
