Job summary

Qualifications

• Registered Nurse with NMC Registration.
• Experience in elderly care beneficial.

Responsibilities

• Assess care and health education needs.
• Develop and evaluate individualised care plans including discharge planning.
• Clinical practice, facilitation and teaching; management and leadership as appropriate.
• Mentor, preceptor, facilitator and role model to junior staff.
