Job summary

University Hospitals Of Leicester NHS Trust in Lincoln is seeking a Staff Nurse to join the Lincoln Renal Community Team. The post involves supporting the development and ongoing provision of care for a delegated caseload, including training for PD and home HD, and education for CKD patients within the renal community setting.

The role requires assessment and delivery of evidence-based nursing care, with participation in team management and organisation to improve patient outcomes and equality

Responsibilities

• Assess, plan, implement and evaluate programmes of evidenced-based nursing care for a cohort of patients.
• Assist and contribute to the management and organisation of the Renal Community Team in clinical and community settings.
• Provide training support for peritoneal dialysis (PD) and home haemodialysis (HHD).
• Educate chronic kidney disease patients and other renal population groups.

Job description

Staff Nurse Within The Lincoln Renal Community Team

The post holder will be responsible for supporting the development of and maintaining the provision of care to a delegated caseload of patients, which may include: peritoneal (PD) and home haemodialysis (HHD) training, supporting patients at home who perform their own PD or HHD, participating in the education of chronic kidney disease patients and other patients the Renal Community Team are responsible for.

Responsible for assessing, planning, implementing and evaluating programmes of evidenced-based nursing care to a group of patients.

Assisting and contributing in the management and organisation of a the Renal Community Team in the clinical area and community setting area.
