Job Details Specialist Support Worker - Fulham, London We're looking for a kind, compassionate and resilient Specialist Support Worker to join our Mental Health Social Care Service in H&F Mental Health. £29,.99 per annum, working 40 hours per week.

What you’ll do


  • Medication Observance: support people who are struggling with medication concordance between the hours of 8am to 8pm, 7 days a week.

  • Complete home visits to observe customers take medication in the borough of Lambeth – Brixton, Streatham, West Norwood, Kennington, Clapham, Oval, and Vauxhall.

  • Support people who are stepping down to more independence, whether from registered to lower support accommodation or returning to their homes after prolonged hospital admission.

  • Identify barriers to medication concordance and achieve goals toward medication self‑management.

  • Connect individuals, as appropriate, with other opportunities of social inclusion and support as part of the Living Well Network.

  • Develop a collaborative discussion with customers on how they are supported to receive medication.

  • Enable customers to improve their overall wellbeing, maintain good physical and mental health, motivate participation in social activity, and help customers identify goals.

  • Jointly work cases with mental health professionals and social workers within the localities.

  • Signpost people to other community services and identify local community resources that could benefit them.

  • Safeguarding – raise alerts within the locality and support investigation or information gathering as required.

  • Ensure timely recording of client concern or safeguarding issues.
