Job description

Are you looking for an autonomous role where you can help shape the future of our community same day services?

Are you passionate about helping to avoid unnecessary hospital admissions and caring for patients in their own homes?

Are you interested in joining us as a Specialist Practitioner (SP)?

This key role provides high-quality care for people in crisis within a 24-hour period, aiming to prevent hospital admissions where safe to do so. You will deliver patient care at the patient's residence, in Same Day Care Emergency Units in Abingdon, Witney, and Henley, and assist in triaging urgent referrals into our Single Point Access (SPA).

This role offers a unique opportunity to work within our community services to test, inform, and refine the development of our same-day care services.

Our services operate on a shift basis, seven days a week, from 8 am to 8 pm. Flexibility to work evenings, weekends, and bank holidays is required. We are open to flexible and part-time hours, with a minimum of 30 hours per week, or full-time at 37.5 hours per week.

(This role is subject to job evaluation, and duties may evolve.)

Key Responsibilities:
  • Manage undifferentiated and undiagnosed conditions, developing advanced clinical skills within your competence.
  • Support treatment and management decisions following comprehensive telephone triage.
  • Provide holistic patient care, from assessment to diagnosis, initial treatment, and discharge, including prescribing and administering medication.
  • Collaborate within the multidisciplinary team and with external providers to ensure timely and appropriate care.
  • Respond rapidly to crisis situations to help individuals maintain or regain independence and prevent unnecessary hospital admissions.
