Overview
We have an exciting opportunity for rehab focussed Occupational Therapists to join the Community Division in the Wigan borough.
The role is part of the new Intermediate Care at Home Team delivering assessment and rehabilitation for patients in their own place of residence and other community settings, supporting safe discharge through a Home First approach.
Responsibilities
- Complete functional and specialist Occupational Therapy assessments in community settings to facilitate earlier discharges and support people to remain in their own homes and improve functional abilities.
- Plan and implement effective interventions, work closely with community services, and effectively manage risk.
- Act as a source of knowledge on the management of a varied range of conditions, providing advisory service to patients, relatives, healthcare professionals and social care.
- Assist the Team Leader in planning, coordinating, delivering and evaluating the intermediate service on a day‑to‑day basis.
- Function as an effective member of the multi‑disciplinary team and the integrated team within Health and Social Care.
- Manage a defined caseload using evidence‑based, client‑centred principles to access, plan, implement and evaluate interventions.
- Regularly supervise students on practice placement, and provide leadership and clinical supervision for other junior staff as delegated by supervisor.
- Support team leaders in the development and delivery of in‑service training for MDT colleagues.
- Consider and support service development alongside senior colleagues, including data collection, audits, service review, etc.
- Provide therapy intervention to patients in a defined caseload using evidence‑based principles.
Qualifications
Essential
- Diploma/Degree in Occupational Therapy
- HCPC Registration
- Post‑registration experience in rehabilitation and condition management
- NHS Experience
- Experience of working with multi‑agencies and an inter‑disciplinary team, and independently
- Experience of supervising students and staff from different professions
- Evidence of working without direct supervision and using own initiative
- Excellent verbal and non‑verbal communication skills, with the ability to motivate and empathise with people with long‑term conditions
- Good literacy in computer use (Microsoft Word, Excel, PowerPoint)
- Effective written and verbal communication, with flexibility in approach
- Excellent time‑management and prioritising skills, ability to organise own caseload and support staff
- Good analytical judgment skills, high level clinical reasoning skills and problem‑solving skills
- Specialist knowledge in relevant clinical skills and patient management, evidence‑based practice, research/audit
- Ability to travel independently within the borough and work flexibly to meet service needs.
Desirable
- Evidence of Post‑Graduate Study and relevant postgraduate qualifications
- Training from a higher education institution for graduate student supervision
- Post‑graduate experience related to clinical speciality and working in a community setting
- Experience in supervising therapy support staff and training staff
- Experience of working in a community setting.
Working Pattern
Monday to Sunday. Full‑time, part‑time, flexible working. Enhanced DBS with Adults’ Barred List required.
