This post will be mainly based at County Hospital (Ward 15) and provide senior medical cover and clinical leadership within Elderly Care, working with a team of Consultant Geriatricians. There will be rotations between County Hospital and Royal Stoke Hospital.
County Hospital, which is part of UHNM, has one elderly care ward in addition to AMU and general medical wards. It provides excellent diagnostic and outpatient and surgical services.
Ward 15 at County Hospital is a purpose-built elderly care ward with 28 beds and currently has one substantive Geriatrician and a locum consultant, supported by a team of training and non-training doctors and advanced clinical practitioners. The ward is also supported by an MDT team comprising physiotherapists, occupational therapists, mental health, and social teams.
There is a daily consultant-led ward MDT board round, followed by a ward round and also a weekly MDT meeting.
County Hospital also has a Frailty Front Door team comprising advanced clinical practitioners who assess frail patients in the ED and facilitate discharge if appropriate.
There will be opportunities for the post holder to undertake sessions or rotate to the Royal Stoke site, as well as opportunities for interested and suitable candidates to take part in acute medical on calls at County Hospital.
Geriatric Medicine at UHNM (Royal Stoke & County Hospitals)
The Consultants in Geriatric Medicine provide services for Older People within the UHNM.
Royal Stoke is the main site, where there are 5 acute Elderly Care wards and a Frail Elderly Assessment Unit (FEAU), and one Elderly Care ward at County Hospital.
Acute Geriatric Medicine
At Royal Stoke, patients who meet the frail elderly criteria are transferred from the ED to FEAU – a 29-bedded unit. Patients are reviewed on the same day or following morning by a Consultant Geriatrician on a post-take ward round.
FEAU is staffed with consultants and junior doctors from Elderly Care from 08:00 to 20:00 hours and covered by the roving medical registrar and a medical SHO outside of these hours. We also accept patients directly from GPs, or from the Community Single Point of Care and aim to ensure that FEAU is only accepting patients who meet our frail elderly admission criteria. Approximately 25% of the patients seen in our Frail Elderly Assessment Unit are discharged from FEAU and the remainder are transferred to one of the Elderly Care wards.
County Hospital has one Elderly Care ward (Ward 15). Patients are transferred to Ward 15 from AMU. The ward has 1 substantive consultant, 1 specialty doctor, 5 trainee doctors, and 1 ANP.

The job plan will be a prospective agreement that sets out the Consultant's typical working pattern, duties, objectives, and responsibilities for the coming year. Additional programmed activities may be offered to the post holder. Any offer of additional programmed activities will be based on the needs of the service and in line with the Trust objectives and will be reviewed on an annual basis.

In particular, where you manage employees of the Trust, you will be expected to follow the local and national employment and personnel policies and procedures.
You will be expected to ensure that there are adequate arrangements for hospital staff involved in the care of your patients to contact you when necessary.
All medical and dental staff employed by the Trust are expected to comply with the UHNM Trust’s Health and Safety Policies.
