Covering the diverse and extensive rural communities of North Northumberland, this role offers the opportunity to work creatively with individuals, families and communities to promote independence, wellbeing and choice. You will also work closely with NHS colleagues, particularly hospital teams and community health services, supporting effective discharge planning and ensuring people receive the right support at the right time to achieve the best possible outcomes. In this role, you will conduct comprehensive assessments and enjoy a healthy balance of autonomy and support. You'll manage your own caseload while receiving regular supervision and consistent guidance from Advanced Social Work Practitioners and your Team Manager. A clear understanding of the Care Act and the Local Authority's statutory duties is essential.
