Qualifications

• Strong communication and interpersonal skills; able to build rapport with families.
• Excellent organizational skills and ability to manage a diverse caseload.

Responsibilities

• Conducting assessments to understand needs and aspirations of young people and families.
• Developing and implementing personalized support plans for transition to adulthood.
• Collaborating with schools, healthcare providers, and community organizations.
• Advocating for rights and access to resources for young people.
• Maintaining accurate documentation and case records in line with policy.
