Job summary

Leeds City Council is seeking a Senior Social Worker in Leeds to lead learning disability social work within a multidisciplinary team. You will assess risks, devise and review care and support plans, and ensure person-centred approaches that empower adults with learning disabilities and involve families and carers.

You will lead on complex cases, support colleagues, and contribute to service improvement while upholding professional standards and safeguarding duties.

Qualifications

• Senior Social Worker registered with Social Work England.
• Approved Mentor/Practice Assessor
• Practice Educator Level 2 or equivalent, or undertake this on appointment (funded)
• Significant post qualifying experience
• Knowledge of learning disability practice and MDT collaboration
• Experience in person-centred assessment and care planning

Responsibilities

• Manage a complex caseload of adults with learning disabilities, undertaking assessments, reviews, care and support planning, safeguarding enquiries and risk management.
• Lead on complex casework involving high risk, safeguarding, mental capacity and legal interventions.
• Develop, implement and review person-centred care and support plans to promote independence and wellbeing.
• Coordinate safeguarding enquiries and investigations with professional colleagues and partners.
