Job summary

Middlesbrough Council is seeking an experienced Learning Disabilities Social Worker to join the adult services on an ongoing contract. You will manage a varied caseload, undertake Care Act 2014 assessments, determine eligibility for services, and develop strengths-based support plans in collaboration with a multidisciplinary team.

The role will involve Mental Capacity Act assessments and best interests decisions, plus Section 117 aftercare planning and trauma-informed risk management.

Learning Disabilities Social Worker - Middlesbrough Council

Service Care Solutions are recruiting for an experienced Learning Disabilities Social Worker to join Middlesbrough Council on an ongoing contract. Rate of Pay: £33.00 per hour Ltd (Umbrella) £25.92 per hour PAYE (exclusive).

This is an excellent opportunity to work within a specialist service supporting adults with learning disabilities, complex mental health needs, offending histories, and individuals at risk of involvement with the criminal justice system. The successful candidate will manage a varied caseload and lead complex social work interventions, working closely with a multidisciplinary team including psychiatrists, psychologists, nurses, and criminal justice professionals.

You will undertake Care Act 2014 assessments , determine eligibility for services, develop strengths-based support plans, and complete Mental Capacity Act 2005 assessments and best interest decisions where required. You will also play a key role in Section 117 aftercare planning , supporting the discharge of individuals from inpatient and secure settings back into the community. The role involves assessing and managing risk, responding to crisis situations using a trauma-informed approach, and providing specialist social work input into safeguarding and care planning processes to achieve positive outcomes for vulnerable adults.
