Hospital Pathway and Reablement (Start) Service

Our hospital team works closely with NHS colleagues to facilitate safe, timely discharges. Their priority is to enable people to return home, utilizing reablement and other services to help them regain independence.

About You

This position offers an excellent opportunity for individuals with outstanding communication skills and a compassionate manner capable of managing high emotional distress. We encourage applications from candidates with experience in adult care and a passion for person-centered care. You will join a welcoming, supportive, and energetic team dedicated to professional development.

In addition, we expect candidates to have:

  • Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
  • At least 2 years post-qualifying experience in adult services, including assessments and support for diverse needs, with successful completion of your ASYE.
  • Knowledge of relevant legislation, including the Care Act and Mental Capacity Act, and social work theories.
  • Strong assessment, care planning, and risk management skills.
  • Ability to communicate complex social issues clearly to patients and professionals.
  • Ability to plan and coordinate social care and discharge plans.
