An exciting opportunity is now available for the correct Social Work candidates to join a hybrid social care services supporting patients in the acute setting and those patients placed to Discharge to Assess & Long-Term facilities.
This role is based over two facilities, one acute and one community which both support multidisciplinary working. As part of this multidisciplinary approach, you will work hand in hand with therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay.
Here at the Northern Care Alliance, the social care teams work 37 hours weekly which covers 7 day working via a team rota which includes days, evenings, and weekends.
As a Social Worker in the Hospital Discharge Team & Community Home First Hub Service, you would have the following responsibilities:

This support includes access to specific programmes, e-learning, training courses, shadowing opportunities, partnership-led workshops, and bespoke CPD funding to help you access university modules linked to your goals and objectives to take your career in the right direction.
