Qualified Social Worker, Integrated Hospital Discharge Team

Pay rate: £35 per hour

Vitalis is recruiting for a qualified social worker to work in an Integrated Hospital Discharge Team in St Helens.

The role involves assessing patients and facilitating hospital discharges to support patients in returning home or moving to another setting. It is a fast-paced, high-pressure environment, and experience in this area is preferred. The position is full-time (37 hours per week) with working hours from 8:45 am to 5:15 pm. The role involves a rota of office work, remote work, and ward visits as necessary.
