Qualified Social Worker, Integrated Hospital Discharge Team

Pay rate £35 per hour

Vitalis is recruiting for a Qualified Social Worker to work in an Integrated Hospital Discharge Team in St Helens.

Experience in assessing patients and facilitating hospital discharges is required. The role involves supporting patients to return home or to another setting, and includes ward visits and assessments. It is a fast-paced, high-pressure role. The position is full-time (37 hours/week), with hours from 8.45am to 5.15pm, involving a rota of office and home working.
