We are seeking to recruit an experienced Social Worker to join the Hospital Discharge Team, based at Chesterfield Royal Hospital.

This is a fixed term post to cover maternity leave . Experience of working in a hospital setting is desirable although not essential, you will need to have transferrable skills to enable you to facilitate safe, sustainable transfers of care for people leaving hospital.

This post will require you work flexibly across hospitals north of the County to maintain system flow as required.

You will need to be able to manage change and have the ability to maintain positive relationship with health partners and other agencies. You will need to be committed to working in a person centred way with people and their families to maintain and maximise their independence, whilst observing the tight timescales required within the Hospital Discharge Guidance.
