Job Description:

I am looking for an Adult's Social Worker to join a Hospital Discharge Team.

Requirements:

• Social Work qualification with a minimum of 3 years post-qualification experience.
• Enthusiastic and committed with substantial knowledge in an Adult's setting.
• Knowledge of key legislation and working in accordance with it.
• Degree within Social Work (Degree/DipSW/CQSW).
• Valid Social Work England registration.
• Full UK driving license preferred.

This is a fast-paced team ensuring patients are safely discharged from hospital with adequate support. Advocating for service users and their independence is a priority. Use of D2A procedures is a daily necessity.
