Job summary

Coventry City Council's Housing and Homelessness Service is seeking a Social Worker to undertake Care Act assessments, develop strengths-based reviews, manage a complex caseload and create outcome-focused support plans for adults with care and support needs. You will work assertively to engage individuals, help them sustain accommodation, improve wellbeing, reduce risk and achieve greater independence.

Qualifications

• Strong understanding of Adult Social Care legislation
• Experience working with adults with complex needs e.g. homelessness
• Ability to engage with individuals who may be difficult to reach or reluctant to access services

Responsibilities

• Undertake Care Act assessments and complete strengths-based reviews
• Manage a complex caseload and develop outcome-focused support plans
• Coordinate support by working with housing providers, health services, substance misuse teams, community organisations and Adult Social Care colleagues
• Advocate for individuals and safeguard vulnerable adults to achieve positive outcomes
