Job summary

Qualifications

• A recognized social work qualification (e.g., Degree in Social Work or equivalent).
• Current registration with Social Work England.
• Proven experience in adult social work, particularly within community settings.

• Excellent organizational skills and the ability to manage a caseload effectively.

Responsibilities

• Conducting comprehensive assessments of adults' needs and risks.
• Developing and implementing personalized care plans in collaboration with service users and their families.
• Providing ongoing support and monitoring of care plans to ensure effectiveness.
• Collaborating with multidisciplinary teams and external agencies to facilitate service delivery.
• Maintaining accurate records and documentation in accordance with legal and organizational standards.
