Job summary

Caritas Recruitment is seeking a dedicated social worker to join our Somerset team and help adults access compassionate support and services.

You will conduct assessments, develop care plans, advocate for clients, collaborate with healthcare and community partners, and maintain accurate case notes. We offer flexible hours, remote work opportunities, ongoing professional development, and a supportive team.

Qualifications

• A recognized social work qualification (Degree in Social Work, CQSW, or equivalent).
• Current registration with Social Work England.
• Experience in adult services with knowledge of relevant legislation and best practices.
• Excellent communication and interpersonal skills.
• Strong organizational and time-management skills.

Responsibilities

• Conducting comprehensive assessments to determine the needs and circumstances of adults requiring support.
• Developing and implementing tailored care plans in collaboration with clients and their families.
• Providing ongoing support and advocacy to clients, ensuring their voices are heard and respected.
• Collaborating with multidisciplinary teams, including healthcare providers, community organizations, and other stakeholders.
• Maintaining accurate and timely documentation of case notes and progress reports.
