Job summary

You will conduct comprehensive assessments, develop tailored care plans, and maintain case records while collaborating with healthcare professionals and community organizations.

Qualifications

• A recognized social work qualification (e.g., Degree in Social Work or equivalent).
• Registration with Social Work England.
• Proven experience in adult social work, particularly in community settings.
• Strong assessment and analytical skills, with the ability to develop effective support plans.
• Excellent communication and interpersonal skills, with a compassionate approach to client interactions.

Responsibilities

• Conducting comprehensive assessments of adults in need of support, identifying their strengths and needs.
• Developing and implementing tailored care plans in collaboration with clients and their families.
• Providing ongoing support and intervention, ensuring the wellbeing of clients is prioritized.
• Working closely with multidisciplinary teams, including healthcare professionals and community organizations.
• Maintaining accurate and up-to-date case records and documentation in compliance with relevant legislation.
