Job Title: Social Worker - Community Team - Cheshire East
Location: Cheshire East
Pay Rate: £35

Liquid Personnel is currently recruiting for a Social Worker to join a Community Team based in Cheshire East.

What will your responsibilities be?

• Conduct comprehensive assessments to determine the needs and risks of individuals.
• Develop and implement care plans in collaboration with service users, families, and multidisciplinary team's
• Provide guidance and support to individuals, empowering them to access services and maintain independence.
• Advocate on behalf of service users to ensure their rights and needs are met.
• Maintain accurate records in compliance with local policies and legislation.
• Work closely with healthcare professionals, voluntary organizations, and community groups to coordinate care.
• Ensure safeguarding principles are upheld, identifying and responding to concerns appropriately.

Qualifications and Experience:

To be successful in this role you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
