We are looking for a dedicated and highly motivated team player to join our welcoming, experienced, and proactive multi-disciplinary Community Access Team in Adult Social Care.

The community access team has an emphasis on an integrated approach, working creatively to achieve positive outcomes, ensuring that care is delivered in a dignified and respectful way. As such you will work alongside Social Care Assessors, Occupational Therapists, Occupational Therapy Assessors to ensure the most appropriate route is accessed for residents to receive appropriate care and support considering the most up to date services, equipment, technology and techniques are being utilised.

You will give information, advice and guidance whilst helping to promote wellbeing and independence and preventing, reducing, or delaying the need for services. When necessary, you will ensure that the people using our services move seamlessly through to other teams whilst recording information using the IT systems in place. Therefore, the ideal candidate will require skills such as having a professional manner, be a skilled communicator, be organised and have prior good computer literacy. You will receive training on specific IT systems used.

You will require a thirst to learn about the up-to-date available services in the community such as voluntary agencies and charitable services. Support will be given to expand on your community knowledge; however, some prior knowledge is desirable.

We are looking for dedicated and highly motivated team players to join us as Social Workers working in line with the Care Act . Our Social Workers play a critical role in supporting people to develop creative and innovative solutions to meet the outcomes they want to achieve.

You will have the opportunity to;

  • Experience working with people with Learning Disabilities and or Mental Health needs and an understanding of the policy context surrounding these areas of practice.
  • Understanding of the complex working areas of forensic case work
  • Understanding of complex health and care needs

You will have:

  • CQSW/CSS/Diploma/Degree in Social Work or equivalent (Essential)
  • Social Work England Registration (Essential)
  • Be required to undertake an enhanced DBS plus Adults Barred List (Essential)
  • Statutory and policy base of Social Work (Essential)
  • Appreciation of recent developments in service provision.
  • Ability to communicate effectively with customers, carers and families.
  • Ability to cope positively with change and an ability to promote positive changes to make a difference.
  • Awareness of importance of multi-disciplinary working and an integrated approach.
  • Ability to carry out assessment and support planning in line with relevant legislation.
  • You are confident, energetic and enthusiastic as well as sensitive and resilient.
  • Self-motivating individual with the passion to make a difference in adult social care.
  • Resilient personal stress management.
