Job summary

We are currently recruiting for an Experienced Social Worker to join the Rhondda Fawr Care & Support Team in Rhondda Cynon Taf.

This is an excellent opportunity for an experienced, qualified Social Worker to join a supportive and well-established team, working alongside an experienced management structure and a range of partner agencies.

The Role

As an Experienced Social Worker, you will manage a defined caseload and undertake high-quality assessments, care and support planning, risk management and review work. You will work collaboratively with individuals, families, colleagues and partner agencies to achieve positive outcomes and promote independence.

You will be expected to demonstrate a strong understanding of relevant legislation and best practice, including the Social Services and Well-being (Wales) Act, Mental Capacity Act, safeguarding procedures and anti-oppressive practice.

Key Responsibilities

• Manage a varied and defined caseload effectively.
• Complete comprehensive assessments and develop appropriate care and support plans.
• Identify and manage risk, including safeguarding concerns.
• Apply relevant legislation and professional standards to practice.
• Work collaboratively with health, social care and other partner agencies.
• Promote strengths-based, person-centred and preventative approaches.
• Maintain accurate and timely case records.
• Participate in reviews and multi-disciplinary meetings.
• Support individuals to maximise independence and achieve positive outcomes.
• Contribute to the continued development of effective social work practice within the service.

About You

To be considered for this position, you will need:

• A recognised Social Work qualification.
• Minimum 3 years' post-qualifying Social Work experience.
• Current registration with Social Care Wales.
• Strong assessment, communication and care-planning skills.
• Sound knowledge of relevant Welsh social care legislation.
• Experience managing a caseload within a social care setting.
• The ability to work effectively with partner agencies and other professionals.
• A commitment to anti-oppressive, person-centred and strengths-based practice.
• Excellent organisational and interpersonal skills.

An enhanced DBS check will be required for the successful candidate.
