Caerphilly County Borough Council is seeking a Social Worker to join the Hospital Discharge Team. The role focuses on supporting Caerphilly residents to achieve safe and timely discharge from hospital within the Hospital Discharge Service, part of the Community Resource Team.
You will need a Diploma/ Degree in Social Work and be registered with Social Care Wales (or eligible to transfer). A full UK Cat B driving licence is required to travel to sites across the borough.

We are seeking a Social Worker to join our Hospital Discharge Team, supporting Caerphilly residents to achieve safe and timely discharge from hospital. The role sits within the Hospital Discharge Service, part of the wider Community Resource Team (CRT) - a large multi-disciplinary service working alongside OTs, Physios, Pharmacy, Rapid Nursing, and the in house domiciliary care service. This role involves working in a fast-paced hospital environment with health and social care colleagues.
