Job summary

Leaders in Care are supporting a North Wales local authority to recruit an experienced Social Worker for the Children with Disabilities Team on a long-term contract. The role focuses on relationship-based practice, high-quality assessments, and tailored care plans to improve outcomes for children with complex needs and their families.

You will join an experienced team with strong management support, regular supervision, and a stable service.

Qualifications

  • Qualified Social Worker with registration in Social Work England.
  • Experience in Children with Disabilities, Safeguarding, Child in Need, or Assessment teams.

Responsibilities

  • Manage a caseload of children with disabilities and complex needs.
  • Undertake Child and Family Assessments and statutory assessments.
  • Complete Care Act and Short Break assessments where appropriate.
  • Develop, implement and review child-centred care and support plans.
  • Collaborate with families, schools, health professionals and partner agencies.
  • Assess and manage safeguarding concerns per statutory guidance.
  • Support looked after children and safeguarding processes as required.
  • Prepare high-quality reports, assessments and court documentation when necessary.
  • Ensure the voices of children and young people remain central to planning.

About the Service

Key Responsibilities
  • Manage a caseload of children and young people with disabilities and complex needs
  • Undertake Child and Family Assessments and statutory assessments

  • Work closely with families, schools, health professionals, and partner agencies

About You
  • Qualified Social Worker
  • Registered with Social Work England
  • Recent experience within a Children with Disabilities, Safeguarding, Child in Need, or Assessment Team
  • Strong knowledge of legislation relating to children with disabilities and safeguarding
  • Experience working with children and young people with complex needs
  • Excellent assessment, care planning, and relationship‑based practice skills
  • Ability to build effective partnerships with families and multi‑agency professionals
