Social Workers play a vital role within Adult Social Care, ensuring that individuals have a solution-focused plan to achieve their goals and maintain their independence. They focus on individuals’ strengths rather than limitations and support them in improving their health and wellbeing.
They work as part of an integrated multi-disciplinary team to provide high-quality social work practice.
They deliver expert and effective social work in complex situations, ensuring that practice is planned, progressed, and reviewed according to legislative and departmental requirements.
They support staff and team development to promote an approach based on co-production principles. They also promote and develop integrated, collaborative working to achieve the best outcomes. This includes using Connect to Support to help people access information, advice, and voluntary and community sector organizations.
They manage high levels of risk, providing expert opinions on complex cases and supporting staff through supervision and performance monitoring.
