What will your responsibilities be?
In this role, you will conduct social care assessments and arrange timely care packages for hospital inpatients prior to discharge. You will also complete Mental Capacity Act (MCA) assessments, make best interest decisions, manage safeguarding responsibilities, and demonstrate knowledge of Section 117 of the Mental Health Act 1983 (MHA), participating in related meetings and discussions. Additionally, you will contribute to the duty rota and collaborate with multidisciplinary teams to ensure safe and effective patient discharge.
