Pay Rate: £ 30/per hour
Liquid Personnel is seeking an experienced Adult Social Worker-Hospital Discharge to join their esteemed client. This is an exciting opportunity for ARCH Social Workers to become part of our team! The role involves facilitating patient transitions, discharge planning, and MDT work. You will also support the Transfer of Care hub and D2A provision.
Responsibilities:
Conduct assessments to identify patients' care needs prior to discharge.
Collaborate with hospital staff, families, and community services to create tailored care plans.
Facilitate access to social care services and resources, ensuring timely intervention and support.
Monitor and review care plans, adapting them as needed to ensure optimal outcomes.
Advocate for patients, safeguarding their rights and well-being throughout the process.
