Job description

To be responsible for both the assessment of individual needs and the initiation and co-ordination of a range of outcomes to meet them, promoting the principles of a strength-based approach, personalisation, rights-based social work practice, choice, and control, to ensure that people can assess and manage their own needs, risks, and uncertainties.

Job responsibilities

Nottinghamshire County Council is developing an all-age Mental Health offer, alongside its existing work for people with Learning Disability and Autism. This will involve strengthened multi-disciplinary working with various partners. The aim is to promote best practice, manage demand, and respond to legislative change, with a focus on reablement. We want the people we serve to live good, ordinary lives in the place they call home, with the people and things they love, in communities where they look out for one another. If people need support, we start by understanding what a good life looks like for them, and how we can work together to achieve it. We are looking for dynamic staff with relevant knowledge, skills, and expertise in Mental Health, Learning Disability, Autism, and Forensic Social Work, to facilitate hospital discharge. Responsibilities include:

  • building trusted relationships
  • having conversations with people based on what matters most to them
  • making sure people's wishes, feelings, and beliefs are central to decision-making
  • supporting people to live as independently as possible, for as long as possible
  • connecting people to wider networks of opportunity and support
  • increasing people's hope, self-esteem, and creative potential

The role ensures the delivery of high-quality, person-centred support and involves working with partner organisations to shape and improve Mental Health, Learning Disability, and Autism services for the community. Essential requirements include:

  • recognising that people are the experts in their lives
  • communicating respectfully with and about people
  • thinking and acting creatively
  • challenging circumstances that marginalise, exclude, or oppress
  • critical reflection and a desire to keep learning

This is an exciting time for Adult Social Care as new operational arrangements are implemented, focusing on equity, individual outcomes, independence, and partnership working. You will contribute to transforming wider Adult Social Care services in line with the Council's strategic priorities and embed our Owning and Driving Performance Programme.
