Job summary

Haringey Council in London is seeking a passionate Social Worker to join our Adult Social Care team at St Ann’s Hospital. You will manage a varied caseload, undertake Care Act assessments, safeguarding enquiries, and hospital discharge planning.

This is a permanent, 36-hour-per-week role with progression from PO2 to PO4, and a starting salary of £43,680–£52,194 plus a £1,000 market supplement. You must be registered with Social Work England and able to work collaboratively with partners to

About The Role

We are looking for a passionate and committed Social Worker to join our Adult Social Care team. In this rewarding role, you will manage a varied caseload, undertaking Care Act assessments, safeguarding enquiries, mental capacity assessments, hospital discharge planning and person-centred support planning. You will work collaboratively with individuals, families, carers, health professionals and community partners to promote independence, reduce risk and achieve positive outcomes for residents. The role offers opportunities for professional development and progression from PO2 to PO4 based on experience, performance and professional development

About The Team

Our Adult Social Care service is dedicated to delivering high-quality, strengths-based support that puts residents at the heart of decision-making. Working within a supportive and collaborative environment, you will be part of a multidisciplinary team alongside Social Workers, Senior Practitioners, Team Managers, health colleagues and partner agencies. We are committed to continuous learning, reflective practice and professional development, ensuring our teams have the support and opportunities they need to deliver outstanding services to our diverse communities.

About You

To be successful in this role, you must hold a recognised Social Work qualification and be registered with Social Work England. You will have experience of working with adults and their families, including within culturally and linguistically diverse communities. You will be able to undertake high-quality assessments, analyse complex information and develop effective care and support plans within statutory timescales.

We are looking for someone with a strong understanding of relevant legislation, including the Care Act, Mental Capacity Act, Mental Health Act, Human Rights and Equality legislation, alongside a commitment to strengths-based practice and promoting independence. Excellent written and verbal communication skills are essential, with the ability to produce clear reports, respond to enquiries and complaints and work effectively with a range of professionals and stakeholders
