Job summary

Hampshire County Council is seeking an experienced Social Worker to join the Intermediate Care Team in Hampshire. You will support individuals with mental health needs, assist with hospital discharge, and review Section 117 aftercare requirements, delivering strengths-based interventions to promote independence in the community.

The role offers hybrid working across Countywide Hampshire, with opportunities for supervision and professional development.

The Role:

Joining our Intermediate Care Team as a qualified and experienced Social Worker, you will play an integral role in supporting individuals with diverse and complex mental health needs. The team works closely with Mental Health Hospital Teams and Joint Commissioners to facilitate timely hospital discharge, identify and review eligible Section 117 aftercare needs, and promote independence through strengths-based interventions. ICT typically support for up to 4 to 6 weeks post discharge, ensuring individuals have the right support in place to achieve successful community outcomes.

What you’ll do:
  • Engaging in comprehensive assessments and reviews of individuals in the community who have eligible s.117 aftercare needs.
  • Supporting the hospital discharge planning process through timely and effective assessments.
  • Managing a diverse caseload, providing guidance and supervision to Case Workers and Senior Case Workers, and offering expert advice on complex cases.
  • Collaborating in a multi-disciplinary manner, effectively negotiating with individuals, their families, and other professionals to achieve positive results.
  • Triaging safeguarding referrals and making well-informed decisions to ensure the best possible outcome.

  • Social Work qualification and registered with Social Work England (SWE).
  • A sound understanding of how to apply a strengths-based approach.
  • Understanding of social work issues, legal frameworks and social services’ statutory duties.

Travel Requirements:
