Job summary

Nottinghamshire County Council's Adult Social Care is seeking a Social Worker to join the Mental Health Discharge Team. In a fast-paced role, you will support individuals in acute psychiatric wards, ensuring timely, person-centred discharge planning and coordinated care from hospital to home.

You will work with health and social care partners to enable safe discharges, undertake strengths-based assessments, and promote reablement and community-based support, contributing to better outcomes for

Qualifications

  • A recognised Social Work qualification.
  • Registration (or eligibility to register) with Social Work England.
  • Knowledge of the Mental Health Act and experience working with mental health.
  • Understanding of community care services and relevant legislation in adult social care.

Job Description
