Job summary

Derbyshire County Council’s North East Derbyshire Adult Social Work Team in Clay Cross, Chesterfield, invites applications for a part-time, registered social worker. The post supports Wednesday to Friday, delivering social work services in the community.

You will undertake Care Act 2014 assessments and reviews for adults with complex needs, coordinate services, and work with partners to promote independence.

Job description

The North East Derbyshire Adult Social Work Team are looking to recruit a part time qualified, registered and experienced full time social worker based at Clay Cross, Chesterfield. The required working patter for this post will be Wednesday, Thursday and Friday.

This is a busy team based in the community and consists of Social Workers and Social Care Practitioners who work closely with partner agencies.

The successful applicant will be able to demonstrate the necessary skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to our clients which helps them to achieve their outcomes. Key responsibilities will include carrying out Care Act 2014 assessments and reviews for individuals with complex needs as well as arranging the provision of services to meet eligible needs. It is expected that you will be committed to working in a person centred and strengths-based way with clients and their families to maintain and maximise their independence.

An understanding and experience of safeguarding is also essential as well as experience of undertaking capacity assessments in accordance with the Mental Capacity Act 2005.

There is an expectation that you will act as duty worker as required which will involve triaging new referrals to the team and supporting clients and carers who require a more urgent response.

You will need to be able to manage change and have the ability to work effectively and build positive relationships with health partners and other agencies.
