Job summary

Nottinghamshire County Council in Nottinghamshire is seeking a Social Worker for the Mental Health Discharge Team in Adult Social Care. The role focuses on discharge planning for individuals in acute psychiatric wards, coordinating with health and social care partners to support safe transitions from hospital to home.

The post is permanent, full-time (37 hours), hybrid working, based at County Hall, West Bridgford.

Qualifications

• Recognised Social Work qualification and registration with Social Work England.
• Knowledge of the Mental Health Act and experience in mental health settings.
• Understanding of community care services and adult social care legislation.

Responsibilities

• Conduct strengths-based assessments and discharge planning.

As a Social Worker, Your Key Responsibilities Will Include
