Job summary

Randstad Care is seeking a compassionate Social Worker to join a Hospital Discharge team in Falkirk. You will assess patients, plan safe discharges and coordinate with medical staff, nurses, therapists and community services. You will advocate for vulnerable adults and families, safeguarding duties and maintaining accurate records.

A UK driving license is preferred; we offer flexible hours, weekly payroll, training and support to source accommodation and travel if required.

Qualifications

• Qualified Social Worker with SSSC registration.
• Minimum 3 years hospital discharge experience.
• Valid UK driving licence preferred.

Responsibilities

• Conduct comprehensive social work assessments to identify patients' needs and develop personalised discharge plans.
• Collaborate effectively with multidisciplinary teams, including medical staff, nurses, therapists, and community services.
• Arrange appropriate care packages, including home care, equipment, and financial support.
• Provide information, advice, and advocacy to patients and their families.
• Safeguard vulnerable adults and children, adhering to relevant legislation and policies.
• Maintain accurate records and contribute to service development.
• Work to prevent delayed discharges and promote patient independence.
