Qualifications

• Bachelor’s Degree in Social Work required or a Bachelor’s Degree in a human services field with one year of supervised social work experience in a health care setting, in accordance with federal regulations.

Responsibilities

• Assist Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services as directed.
• Develop, administer, and coordinate department policies and procedures.
• Review policies annually and propose changes as needed.
• Identify medically related social and emotional needs of residents and develop policies accordingly.
• Provide information to residents/families about Medicare/Medicaid and other financial assistance.

Education

Bachelor’s Degree in Social Work

Social work licensure (LSW/LMSW/LCSW or state equivalent)
