Chorley Council and South Ribble Borough Council are seeking a Social Prescribing Link Worker to support residents in Leyland, improving health and wellbeing through local services and community activity. The role involves signposting, partnership work, and maintaining case notes, with some travel around the boroughs.

You will join a Wellbeing & Resilience service, contributing to person-centred support, safeguarding practices, and efficient referrals to partner organisations.

Qualificações

• Experience supporting residents in a person-centred, solution-focused way.
• Works well in a team.
• Collaborates with colleagues and partners to identify gaps in community provision.
• Knowledge of safeguarding and GDPR processes.
• Knowledge and experience using different IT systems.

Responsabilidades

• Support South Ribble residents with issues important to them in an empathetic manner.
• Provide signposting to immediate and longer-term support within the wellbeing service.
• Through partnerships, ensure accurate and timely interventions are delivered by the appropriate service or organisation.
• Deliver cover for outreach drop-ins where needed.
• Maintain accurate case notes and upload to CharityLog promptly.
