What's the role about?
As a Social Care Coordinator for Education, Health and Care Plans (EHCPs), you'll ensure that the social care elements of EHCPs are robust, timely, and aligned with national standards. You'll work with children and young people aged 0-25, including those already open to social work services, ensuring their care needs are accurately reflected. Your work will support the local authority in meeting its statutory responsibilities under the SEND Code of Practice.

A background in social care or family support, understanding of SEND legislation and safeguarding, excellent communication skills, organisation, and adaptability are essential.
